Responsibilities:

• Assist in evaluating clients' needs and developing appropriate service plans
• Support clients with accessing community resources (housing, food, transportation, employment, etc.)
• Document client interactions and maintain accurate case records
• Help coordinate appointments, follow-ups, and referrals
• Participate in home visits or facility visits as needed
• Collaborate with social workers, healthcare professionals, and other service providers
• Perform administrative tasks, including filing, data entry, and compiling reports
• Help advocate for clients' rights and ensure services are delivered equitably
• Maintain client confidentiality in compliance with HIPAA and agency policies

• Bachelor's degree in Social Work, Psychology, Sociology, or related field
• Prior experience in social services or human services is preferred
• Familiarity with California social service systems is a plus
